Travel Forms List
There are a number of forms required as you participate in the travel program. Most of the forms are defined and required by the Indiana Youth Soccer Association or by US Soccer on a national level. We will link directly to the most common outside forms here, to ensure that you have the most current version of the forms. You will require Acrobat Reader to display these files.
Drop/Transfer
This form is utilized anytime a player is to be dropped from your roster, or transferred to another team’s roster. This form MUST be completed in the event of a transfer, and the transferee’s player card secured. Both should be then forwarded to the Club Registrar, who will complete the transaction with IYSA.
Travel Permit
Any time a team desires to travel to a tournament out of the State of Indiana, it must seek permission from the IYSA to do. All USYSA/USSF tournaments, by rule, will require that a visiting team produce such a permit from it’s home organization at registration. Medical Release Form
While our club medical release form is generally accepted at tournaments in Indiana and the surrounding states, some clubs do require specific and notarized Medical Release Forms. The medical release form linked here provides for Notary signature, if so required.
IYSA Medical Release Form (PDF)
Travel Tryout Form
This form will be needed for our staff to know who your child is during the tryout process and to be able to properly notify you of your selection.
Game Format Form
This form provides all of the necessary information about the field size, ball size, length of the game, referee fees, etc.
Reschedule Form
This form is needed to reschedule league games.
