Medical Insurance
As part of fees paid for participating in both the Recreational and Travel Soccer programs, FSC, through the Indiana Youth Soccer Association, provides for medical insurance coverage that supplements a players private health insurance. An outline of the coverages provided is found below, as well as a claim form, and claim contact information. FSC specifically requests that all injuries that occur in the course of playing, spectating, or volunteering on behalf of the club requiring medical attention be reported to the club. Please direct this information to your age appropriate commissioner for the recreational program, or to the appropriate gender commissioner for travel.

Liability Insurance
Official team members, club officers and directors, club coaches, and anyone else volunteering on behalf of the club is provided liability coverage for accidents arising out of club operations or activities.
