Forms and Waivers

Here's where you will find all the important forms and procedures you will need to know once you are accepted into the tournament. Please follow all instructions carefully. Understand that these requirements apply to everyone and are not designed to be a hardship rather they are required to protect everyone and ensure a safe and fair tournament for all. Documents are in PDF document -- see bottom of page to download the free Adobe Reader if you do not already have it.

Some of the forms you will need are:

Waiver of Liability / Participation Roster / Code of Conduct
One Waiver form required per Team
One Parent of each player must sign
Medical Release Form ("Authority to Treat")
One Release for Each Player
Signed by a Parent or Guardian
MUST BE NOTARIZED
Team Volunteer List (FSC Teams Only)
One form per Team

Waiver of Liability / Participation Roster / Code of Conduct:

It is required that at least one parent for each player participating as a member of the team during the Kohl's Cup must sign the Team Liability Waiver / Participation Roster / Code of Conduct form. This includes a parent for every regular team member and a parent of every guest player who will play with your team during the event. If there are siblings on a team, you will have to have a parent signature on the form for each player. This will be matched with the roster to confirm that EVERY PLAYER has a parent signature.

The parent signature on the form form indicates their agreement to abide by the rules and regulations of the event, their agreement to the waiver of liability for the event and all involved, their agreeement to follow the Parent Code of Conduct -- a set of standards which will allow everyone participating to have a safe, fair and enjoyable event.

Medical Release Form ("Authority to Treat"):

Indiana Youth Soccer requires completion of a notarized medical release for all tournament events. We must have one signed and notarized release for each player on the team, both regularn players and guest players. As a coach/manager/team parent, you may print these forms and distribute to the parents so that they may have them signed in the presence of a notary.

If you do not have ready access to a notary public, one of our sponsors, The National Bank of Indianapolis, will provide notary services to help participants meet this requirement for no charge. Team Volunteer List (FSC Teams Only):

To successfully run the tournament, we require the services of many parent volunteers. We are requiring that Fishers Soccer Club teams provide a list of parent volunteers to fill thse positions. Please complete the form and return it as directed.
